Step 1 – Check In

To save time on your next visit, print and fill out the following forms before you arrive:

Step 2 – Visit

After checking in, a member of our staff will take your vitals and the medical provider will care for your diagnosis. Remember to bring the following items:

  • Valid photo ID
  • Insurance card (if you have insurance)
  • Credit, debit card, cash or personal check for co-pay or self-pay fee
  • If you are under 18, you must be with your parent or legal guardian

Step 3 – Check-Out

When your visit is finished, you will receive a record of your visit as well as any aftercare instructions and/or prescription information at the front desk.

Step 4 – Follow Up

Within a few days after your initial visit, you will receive a follow-up phone call asking a few questions to check on your recovery process and inform you if there are any additional checkups needed.

When do I pay for treatment?

Payment is due at the time of treatment, unless an alternative payment has been arranged prior to your visit. Patients will be asked to provide a valid photo ID along with proof of insurance (if you have insurance) upon check-in. Gateway Urgent Care accepts most major credit cards, debit cards, and/or cash.
